This is the case report of hemolytic disease of the newborn(HDN) due to
anti-Jkb.
The mother was a 28 year-old gravida 1, para 1, whose miscarriages history was
doubtful. She had been transfused or injected with blood. She had a full term
spontaneous delivery of a 2.4§¸, female infant.
The infant became jaundiced in the first 24 hours after birth. Her total bilirubin level
was 24.5 §·/dl at 48 hours after birth. The direct Coombs' test on the infant's blood
was positive(+++). The hemoglobin was 13 gm/dl. Moderate anisocytosis and
polychromasia as well as a few sherocytes were seen.
From the mother's serum, an irregular antibody was found and identified as
anti-Jkb An eluate was made from infant's blood and it was unsuccesful
to detect an antibody.
The blood group of the mother was 4, R©ûR©û (DCe), Jkb-positive,
Jkb-negative. The blood group of the infant was A, R©ûR©û(DCe),
Jkb-positive, Jkb-positive. Therefore the
anti-Jkb was thought to be the possible offending antibody in this HDN.
HDN resulting from Jkb incompatibility generally followes a benign
clinical course and the prognosis is ordinarily good. Our patient was fully recovered
without exchange transfusion.
